Northern Italy (probably Venice) ; ca. 1350. [98] leaves ; 42 cm. Written on vellum, with copious red and blue initials and lettering, and a multi-color (including gold) initial on the first leaf. The De regimine sanitatis is incomplete: c.f. letter from Paul Gottschalk Antiquar Berlin, dealer from whom Dr. Cushing purchased the manuscript in 1929, concluding that two leaves at end are wanting.

It was purchased from Hogg (book seller) in May, 1960. First item dated Paris le 20 Jan. 1751 on page 176 ; second, in a different hand, is entitled: "Formulles des Remedis Employés dans l'hopital des Religieux de la Charité de Paris" with initials L.S.N.S.J.C. and date 1752.

In two columns, with initials and chapter headings in red. Description based upon dealer's catalog (H.P. Kraus, Rare Books and Manuscripts) dated 4 June 1965; quoted within are De Ricci's Census and Sarton's Introduction to the History of Science.
